   40. Practitioner to notify change of name or address---(1) Every
 medical practitioner who at any time changes his or her name or the
 address appearing in the register in respect of that person shall, no
 later than 1 month after the change, give notice to the Registrar of the
 new name or address.

   (2) The Registrar shall, on receipt of any notice given under this
 section, amend any entry in the register relating to the medical
 practitioner accordingly.
