Medical Practitioners Act 1995

   48. Surrender of certificate of registration---Any medical
 practitioner---
   (a) Whose name is removed from the register; or
   (b) Whose registration is suspended; or
   (c) On whose registration conditions are placed; or
   (d) Whose type of registration changes---
 shall, within 14 days after the date on which notice of the removal,
 suspension, conditions, or change is given to him or her, deliver to the
 Registrar any certificate of registration held by him or her at the time
 of receiving the notice.
